Atypical Primary Upbeat Nystagmus that Does not Conform to the Alexander¡¯s Law

Abstract
Amplitude of upbeat nystagmus (UBN) is increased during upward gaze and decreased during downward gaze. The authors describe a patient with acute multiple sclerosis who developed primary position UBN with a linear slow phase, in which the amplitude of nystagmus is paradoxically intensified in down gaze and decreased during upward gaze. Brain MRI showed high signal lesions in the paramedian dorsal area of the caudal medulla encompassing the most caudal part of the perihypoglossal nuclei. Clinician should be aware of possibility of caudal medullary lesion in patient with UBN, especially when the amplitude of UBN is increased in downward gaze.
